1. Purpose and Scope of Services
Brevard Home Care of Cocoa Beach provides non-medical home care services to help clients remain safe, comfortable, and supported in their homes. Services may include companionship, supervision, assistance with activities of daily living, light housekeeping, meal preparation, medication reminders, errands, transportation assistance, and other non-medical support as agreed upon in the client’s care plan.
Brevard Home Care of Cocoa Beach does not provide emergency medical services, skilled nursing services, medical diagnosis, medical treatment, medication administration, or services that must be performed by a licensed medical professional unless expressly authorized by law and agreed to in writing.
2. Care Plan and Service Schedule
Services are provided according to the client’s agreed-upon care plan and schedule. The care plan may describe the type of assistance requested, preferred routines, safety concerns, household expectations, and any special instructions provided by the client or authorized representative.
· Schedule changes should be requested as far in advance as possible.
· Caregiver arrival and departure times may vary slightly due to traffic, weather, emergencies, or other circumstances outside the agency’s control.
· Brevard Home Care of Cocoa Beach will make reasonable efforts to provide consistent caregivers, but specific caregiver assignments are not guaranteed.
· If a caregiver is unavailable, the agency may offer a substitute caregiver when possible.
3. Client and Family Responsibilities
The client, family, or authorized representative is responsible for providing accurate information needed to deliver safe and appropriate care. This includes notifying Brevard Home Care of Cocoa Beach of changes in health condition, mobility, medications, home environment, emergency contacts, care preferences, risks, or service needs.
· Provide a safe working environment for caregivers.
· Keep walkways, bathrooms, and care areas reasonably clean and free of hazards.
· Secure pets when necessary to protect clients, caregivers, and visitors.
· Provide supplies needed for care, including gloves, wipes, linens, cleaning supplies, food, and personal care items.
· Communicate concerns promptly so they may be addressed in a timely manner.
4. Billing, Payment, and Fees
Payment is due according to the billing schedule and rate agreement provided to the client. Rates, minimum visit requirements, holiday rates, overtime rates, mileage charges, transportation charges, and other fees should be stated in the applicable service agreement, invoice, or written rate sheet.
· Invoices are payable by the due date listed on the invoice.
· Late payments may result in delayed, reduced, or suspended services.
· The client or financially responsible party remains responsible for payment unless otherwise agreed in writing.
· Brevard Home Care of Cocoa Beach may update rates with written notice.
5. Cancellations and Schedule Changes
Clients are asked to provide reasonable advance notice for cancellations or schedule changes. Late cancellations, missed visits, or failure to provide access to the home may result in a cancellation fee if stated in the client’s rate agreement or service agreement.
Brevard Home Care of Cocoa Beach understands that emergencies and unexpected events occur. The agency will make reasonable efforts to accommodate changes when possible, but requested changes are subject to caregiver availability.
6. Emergency and Medical Situations
Brevard Home Care of Cocoa Beach is not an emergency response provider. In the event of a medical emergency, serious injury, sudden change in condition, fire, safety threat, or other urgent situation, the client, family member, caregiver, or authorized representative should call 911 immediately.
Caregivers may notify the agency and designated emergency contacts as soon as reasonably possible after emergency assistance has been requested.
7. Communication
Clients and authorized representatives should communicate service concerns, schedule requests, care changes, billing questions, and caregiver feedback directly to Brevard Home Care of Cocoa Beach. The agency will make reasonable efforts to respond to non-emergency messages in a timely manner.
· Emergency concerns should be directed to 911.
· Non-emergency concerns should be reported to the agency by phone, text, email, or other approved communication method.
· Clients should notify the agency if a preferred communication method changes or becomes unavailable.
8. Confidentiality and Privacy
Brevard Home Care of Cocoa Beach respects client privacy and expects employees and caregivers to maintain confidentiality regarding personal information, care needs, household matters, financial information, and other private details learned during the course of service.
Client information may be shared when necessary to coordinate care, comply with applicable law, respond to emergencies, process billing, or communicate with authorized representatives.
9. Safety, Conduct, and Home Environment
Brevard Home Care of Cocoa Beach is committed to providing services in a respectful and safe environment. The agency may refuse, suspend, or terminate services if the home environment becomes unsafe, if caregivers are subjected to harassment, threats, abuse, illegal activity, unsafe pets, hazardous conditions, weapons concerns, or other circumstances that interfere with safe service delivery.
Clients and family members are expected to treat caregivers and agency staff with courtesy and respect. Caregivers are also expected to treat clients and families with dignity, patience, and professionalism.
10. Non-Solicitation of Caregivers
Clients, family members, and authorized representatives agree not to privately hire, solicit, or directly employ any caregiver introduced by Brevard Home Care of Cocoa Beach during the service relationship or for a reasonable period after services end, unless otherwise agreed in writing by the agency.
11. Service Limitations
Caregivers may only perform tasks within the scope of non-medical home care services and within applicable legal, safety, and agency guidelines. Caregivers may decline tasks that are unsafe, outside the care plan, outside the caregiver’s training, or not permitted by agency policy.
Examples of services that may be limited or prohibited include medication administration, wound care, medical procedures, heavy lifting without proper equipment, handling hazardous materials, financial transactions not approved in writing, or any task requiring a licensed medical professional.
12. Termination or Suspension of Services
Either party may terminate services by providing written notice, unless immediate termination or suspension is necessary due to safety concerns, non-payment, repeated cancellations, inappropriate conduct, lack of required supplies, inability to provide services safely, or other serious concerns.
Upon termination, any outstanding balance becomes due according to the billing terms provided to the client.
